Semaglutide is the cheaper molecule, everywhere

Every provider in our database that offers both compounded semaglutide and compounded tirzepatide prices semaglutide lower, usually by $20 to $40 per month. The reasons are unglamorous: semaglutide's active pharmaceutical ingredient costs compounding pharmacies less per milligram, and the labeled doses are far smaller — 0.25 to 2.4 mg weekly for Wegovy against 2.5 to 15 mg for Zepbound — so a month's supply contains a fraction of the peptide. The result is that the cheapest GLP-1 plan online is, at any given provider, the semaglutide plan.

This guide ranks compounded semaglutide providers on 12-month all-in cost, in sentences rather than a table, for the same reason our tirzepatide guide does: a table's price column is usually the starting-dose price without the membership, and it produces the wrong ordering.

The cheapest compounded semaglutide, ranked

Thirteen providers now have verified or reported semaglutide prices, ordered below by 12-month all-in total on the same month-by-month model used everywhere on this site. Every verified figure was checked against the provider's own pages on August 25, 2026.

Sunlight advertises flat monthly pricing starting at $88 for compounded semaglutide with prescription and ongoing support included, which annualizes to $1,056 if the price holds at every dose; the site does not say whether it does, so the row carries that assumption as a flag, and Sunlight has not named its pharmacy. Trimi is $99 per month on an annual plan with dose-inclusive pricing ($1,188, paid up front) and names VialsRx and GreenwichRx as its 503A pharmacies; shipping and consultation terms are unpublished.

NexLife is $119 per month on its 12-month plan ($1,428), the lowest semaglutide total with no flags: the same price from 0.25 mg to 2.4 mg and beyond, no membership, clinical care, a nutrition plan, a coaching call, 24/7 support and expedited shipping included, billed monthly, with a stated price lock. Its 6-month plan is $125, its 3-month plan $132, and its month-to-month plan $139, the lowest verified month-to-month cost at maintenance doses in the database. NexLife also sells a microdose semaglutide plan at $110 to $129 and a sublingual ODT plan at $165 to $199, both listed and unranked because sub-label protocols and unproven dosage forms are not what the STEP evidence supports.

Ozari Health charges first-time patients about $257 for the first three months ($86 per month), then $435 a quarter ($145 per month) or $175 month-to-month, for a first-year total of $1,563. Orderly Meds is $149 per month, flat, with its five 503A pharmacies named ($1,788). Mochi Health charges $60 for the first month then $99 for medication, plus a membership of $39 for the first month then $79: $99 in month one and $178 a month after that ($2,057). Ivím Health sells $600 for six months of semaglutide ($100 per month) plus a $75 monthly membership ($2,100). Henry Meds is $297 month-to-month, $247 per month on a six-month prepay and $197 on twelve months ($2,364); its sublingual semaglutide ($249, or $179 on a premium program) is listed, not ranked. Fifty 410 is $199 monthly ($2,388). Noom Med is $129 for the first four weeks then $249 per month billed quarterly ($2,868). Zappy Health is $249 month-to-month, less on three-, six- and nine-month commitments, dispensed by The Pharmacy Hub and available everywhere but Hawaii ($2,988). Remedy Meds is $179 the first month then $299, flat across doses, in all 50 states and DC ($3,468). Sprout Health is reported at $199 then $299 ($3,488), pending our own check.

Providers with semaglutide but no rankable price: Eden, whose public pricing contradicts itself ($149 then $249 in a January 2026 article versus $99 medication plus a $39-then-$99 membership in current callouts); Shed, whose product page and terms conflict; Lavender Sky Health, which prices by pharmacy and quantity from about $95 per month equivalent; Found ("from $99" plus a membership from $49); Strut Health; ReflexMD; MyStartHealth; and Hims, Hers and LifeMD, whose former compounded semaglutide offers are no longer on their current pages, which now sell $39-then-$149 memberships with brand medication billed separately. WeightWatchers Clinic and Calibrate are brand-medication programs and are not compounded offers at all.

What "under $100" usually means

A semaglutide price under $100 per month is almost always one of three things. It is a starting-dose price at a provider whose price rises with dose, so the 2.4 mg price is higher. It is a medication price that excludes a membership fee of $50 to $80. Or it is a promotional first month, or a per-month rate on a prepaid six- or twelve-month plan that must be paid in full up front — Hims and Hers have advertised sub-$100 monthly rates on annual prepayment. Each can be a fair deal for the right patient. None is a $100-per-month plan in the sense of paying $100 each month at the dose you will actually take.

The brand-name benchmarks

FDA-approved Wegovy pens through NovoCare Pharmacy were priced at a flat self-pay rate across doses that fell substantially in 2025, with an introductory rate for new patients; the oral Wegovy tablet, approved in December 2025, launched at a lower price. We have entered these as unverified pending re-check, and we do not rank brand channels against compounded plans. But the direction is clear: the gap between FDA-approved semaglutide and compounded semaglutide has narrowed to the point that a patient paying $139 a month for compounded product should look at what the oral tablet costs before deciding. Semaglutide or tirzepatide?

The price difference is small. In the SURMOUNT-5 trial, tirzepatide produced 20.2% mean weight loss at 72 weeks versus 13.7% for semaglutide 2.4 mg, and the share of participants losing at least 25% was roughly double. In SELECT, semaglutide reduced major cardiovascular events by 20% in people with existing cardiovascular disease, an outcome tirzepatide has not yet demonstrated in an obesity population. Semaglutide also has a longer safety record and an approved oral form. At NexLife the tirzepatide premium is $20 per month on the 12-month plan, or $240 per year. What compounded semaglutide is not

Compounded semaglutide is not FDA-approved and has not been reviewed by the FDA for safety, effectiveness or quality. The semaglutide shortage ended on February 21, 2025, with compounding of copies of Wegovy and Ozempic required to stop by April 22 (503A) and May 22 (503B); what remains is compounding for individual patients with a documented need for a different formulation. Compounded semaglutide has a longer record of quality problems than compounded tirzepatide: the FDA has warned about products made with semaglutide sodium or acetate salts rather than the base peptide, and about dosing errors from multi-dose vials.
