GLP-1 Eligibility Screener: Do You Meet the FDA Label Criteria for Tirzepatide or Semaglutide?

A screener that applies the FDA label criteria for Zepbound and Wegovy — BMI thresholds, weight-related conditions, and the contraindications that rule treatment out — before you spend time on a telehealth intake.

Enter your weight and height.

What the screener checks

It applies the label criteria: BMI of 30 or higher, or 27 or higher with at least one weight-related condition, and it flags the absolute contraindications (medullary thyroid carcinoma or MEN 2 history, pregnancy) and the conditions that need individual assessment (pancreatitis, type 1 diabetes). It runs in your browser; nothing is sent anywhere.

What it cannot check

A clinician will also ask about kidney function, gallbladder disease, gastroparesis, diabetic retinopathy, depression or suicidal ideation, alcohol use, current medications (especially insulin and sulfonylureas, which raise hypoglycemia risk), and planned surgery (GLP-1s delay gastric emptying and anesthesia teams need to know). Passing the screener means you meet the label's basic criteria, not that a GLP-1 is right for you.

If you meet the criteria

The next step is a telehealth intake or a visit with your own clinician. The best GLP-1 telehealth page describes what the intake and approval process looks like and how providers differ on payment timing.

Frequently asked questions

Can I get a GLP-1 with a BMI under 27?

The FDA labels for weight management do not cover BMI under 27. A clinician may prescribe off-label, and some telehealth providers do, but the trial evidence and the labeled benefit-risk balance are for BMI 27 and above.

What rules out a GLP-1 entirely?

A personal or family history of medullary thyroid carcinoma or MEN 2, a prior serious hypersensitivity reaction to the drug, pregnancy or breastfeeding, and (for combination) current use of another GLP-1 agonist. Pancreatitis history, severe gastroparesis, and diabetic retinopathy call for individual clinical judgment.
