Best, defined

A "best" list that is really a cheapest list is not useful, and a best list that is really a sponsor list is worse. This one is computed. Each provider receives a score out of 100 from the published rubric: 45 points for 12-month all-in cost relative to the cheapest provider in the field, 20 for pricing transparency (published prices, no dose-based increases, disclosed fees, a price lock), 15 for flexibility (month-to-month option, cancel anytime), 10 for care and access (24/7 support, consultation included, a brand or insurance pathway), and 10 for verification recency. A provider whose price we have not verified receives a provisional score with the cost component incomplete.

Cost dominates the score because cost dominates the decision for most cash-pay patients, and because it is the component providers most often obscure. The other 55 points exist so that a provider cannot win by being cheap and opaque.

What the score says in 2026

The top of the list is held by flat-rate, all-inclusive providers whose advertised price equals their true price and who publish it before sign-up. The middle is held by membership-plus-dose-based providers, which lose cost points at maintenance dose and transparency points for dose-based increases, but keep flexibility points for month-to-month billing. Insurance-first and brand-focused programs score lower on cash cost but earn the care-and-access points for a pathway to FDA-approved medication, and are the right answer for the patients those pathways serve.
